 Room Request Form

To reserve a room, please complete the form below. Fields marked with an * are required fields.

Your request will be routed to the appropriate R25 Scheduler, based on the EVENT TYPE and CAMPUS or CENTER that you specify.

Need to request for more than one date?  Set the start date and end date and click "More Date Options" to set a weekly pattern for your request.  Use the Comments to add any exceptions or special information we need to know.

NOTE: If you need A/V equipment, you must email the Service Desk using the Request For Media Services form and please submit the form 1 business day in advance.   If you need other setup by Facilities, you need to email to your campus Clean It department.

If you are requesting a room change for a credit class please do NOT use this form.  For credit class changes use the Room Change form.  Need an additional room(s) for activities related to a credit class?  Use this form and indicate the class section # (ENG-101-SC01) in the comments field.
